Key message

Over 10% of patients are prescribed a medication to which they are allergic. These adverse drug events may be preventable.

National Standard

National Safety and Quality Health Service Standard 4.7 requires that health services document previously known adverse drug reactions on initial presentation and update this if an adverse reaction to a medicine occurs during the episode of care. Standard 4.7.2 requires that action is taken to reduce the risk of adverse reactions and standard 4.7.3 requires that the organisation reports adverse drug reactions to the Therapeutic Goods Administration.

To comply with this standard, it is recommended that the adverse drug reaction section on the National Inpatient Medication Chart be kept up to date.
