Existing users can access the e-form by following these instructions:
- Log in to the .
- Click on the ‘VINAH’ tab.
- Select the ‘Late Data Request Exemption Form’ link under ‘Context Functions’ (left-hand side of the screen).
- Create the request by providing financial year, health service, VINAH program and reporting month information.
- The next screen will ask for software provider, the issue, details surrounding the issue and an expected resolution date. You have the option of printing the form prior to submission. The print icon is located on the top right-hand corner of the screen.
- Once complete, click the ‘Submit’ button.
