Key messages

  • The MMP is a standardised form that can be used by nursing, medical, pharmacy and allied health staff to record the medicines taken prior to presentation at the hospital and for reconciling patients’ medicines on admission, intra-hospital transfer and at discharge.
  • The MMP form has been designed for use in adult and paediatric patients.

National Standard 

The National Safety and Quality Health Service Standard 4.12.1 requires that a system is in use that generates and distributes a current and comprehensive list of medicines and explanation of changes in medicines.

The National Standards in Mental Health Services requires that mental health services facilitate continuity of integrated care across programs, sites and other related services with appropriate communication, documentation and evaluation to meet the identified needs of consumers and carers.
