Carbapenemase-producing Enterobacteriaceae – surveillance and isolate referral forms

Key messages

  • All suspected and confirmed isolates of CPE from both human and environmental samples are required to be referred to Microbiological Diagnostic Unit Public Health Laboratory (MDU PHL) for confirmatory testing.

CPE isolate referral forms (human and environmental)

The following forms are to be used by diagnostic laboratories when referring any CPE isolates to the Microbiological Diagnostic Unit Public Health Laboratory (MDU PHL). Note that there are different forms for human and environmental isolates.

CPE surveillance data collection forms (Part A and Part B)

The following forms are the CPE surveillance data collection forms. When completed, forms are to be submitted to VICNISS. Part A is to be used for all confirmed CPE isolates (human) whereas Part B is only used when the likelihood of transmission within a healthcare facility needs to be further investigated.
