Home and Community Care Program for Younger People

HACC PYP supports Victorians from birth to 65 years and Aboriginal people from birth to 50 years to improve their health and wellbeing, while maintaining or regaining their independence to live safely in their homes and actively participate in their community.

HACC PYP provides goal-directed supports that respond to individual need, are generally time-limited or intermittent, and targeted to those most at risk. HACC PYP supports are intended to complement informal supports and other parts of the healthcare and community support services system.

HACC PYP services are delivered across Victoria, primarily in home and community based settings, by a diverse range of the following providers:

  • Aboriginal community controlled organisations
  • Community service organisations, including:
    • Registered community health centres
    • Local government authorities
    • Non government organisations
  • Health services, including regional, rural and metropolitan health services.

Accessing HACC PYP

Where possible, people are encouraged to access HACC PYP through an assessment provider. Assessment providers assess a person's needs, goals and circumstances and help connect eligible consumers to appropriate HACC PYP and other services. Some people may also access specific HACC PYP services directly, particularly nursing and allied health services.

HACC PYP Provider Directory

The HACC PYP Provider Directory specifies all the services delivered in each local government area and the providers which deliver them, with links to their websites.

HACC PYP services are available to help people with:

  • Getting started and connecting to services – assessment and access supports
  • Getting allied health care at home – allied health
  • Getting clinical nursing care at home – nursing
  • Getting help with daily living at home – personal and in-home support
  • Getting involved in your community – social and community engagement

Where possible, people are encouraged to access HACC PYP through an assessment provider. Assessment providers can help identify a person's needs, goals and circumstances and connect them with appropriate HACC PYP and other services. Some services, particularly allied health and nursing services, may also be accessed directly.

HACC PYP Guidelines

The HACC PYP guidelines (2026) provide the policy framework for HACC PYP, including service access, delivery, performance, and reporting. The guidelines support funded HACC PYP service providers to deliver high quality services that are person centred, responsive to individual consumer circumstances, and aligned with the objectives of HACC PYP.
