Community health data reporting

Information for community health service providers to understand reporting obligations for funded activities and programs.

The Department of Health (the department) provides funding for a range of community health activities and programs. As a condition of funding, service providers are required to report back to the department about the services they deliver.

The department uses community health data for a variety of purposes, including performance monitoring, service planning and policy development.

The Community Health Minimum Data Set (CHMDS) is the primary data collection and reporting tool for community health activities and programs. The CHMDS is used to collect data about service provision to clients for the following programs:

In some instances, additional reporting is required for specific programs and activities. Service providers should refer to their funding agreement with the department, as well as the relevant program guidelines to understand all applicable reporting requirements.
