Community Health Minimum Data Set (CHMDS)

The Community Health Minimum Data Set (CHMDS) collects information about clients and services provided by agencies funded to deliver the following programs:

  • Autism Assessment
  • Bush Nursing
  • Community Asthma Program
  • Community Health Program
  • Diabetes Connect
  • Family Planning
  • Family and Reproductive Rights Education Program
  • Health Support for Children in Care
  • Healthy Mothers Healthy Babies
  • Infant Child and Family Health and Wellbeing Hubs
  • Innovative Health Services for Homeless Youth
  • Integrated Chronic Disease Management
  • MDC Community Health Nurse
  • Putting Families First
  • Refugee & Asylum Seeker Health
  • Small Rural Health Services – Primary Health

Submission guidelines

The CHMDS quarterly data submissions due dates are:

  • Quarter 1 - July to September: due 15 October
  • Quarter 2 - October to December: due 15 January
  • Quarter 3 - January to March: due 15 April
  • Quarter 4 - April to June: due 15 July

Annual changes

Each year the department reviews its key health data collections to ensure the data collected:

  • supports the department's state and national reporting obligations
  • assists service planning and policy development
  • reflects changes in funding and service provision arrangements for the coming financial year
  • incorporates appropriate feedback from data providers on improvements.

The annual change process is managed by the department’s eHealth Division with stakeholder consultation through the CHMDS Change Management Group.

Bulletins

The CHMDS bulletin provides information and updates on a range of issues including data quality, data collection and submission, and changes to the dataset.

The bulletin is produced on an ad hoc basis. To be added to the distribution list, please send an email request to chmds-data@health.vic.gov.au
