Seniors Lifestyle Victoria

Seniors Lifestyle Victoria supports older Victorians to navigate the healthcare system so they can stay active, stay strong and stay connected.

The Seniors Lifestyle Victoria program offers access to a range of services to support participants to stay independent and well. This includes support with physical activity, nutrition, medication management, and social connection.

A coordinator will work with the participant to understand their health and wellbeing needs and develop a plan that supports their goals.

Participation in the Seniors Lifestyle Victoria program is free.

The Seniors Lifestyle Victoria program is a pilot

This program is funded by the Australian Government as part of the Strengthening Medicare - Supporting Older Australians package. This package is focused on reducing avoidable hospital presentations and admissions and improving the overall health and healthcare outcomes of older Australians.

Seniors Lifestyle Victoria is a program that will be available through selected community health services in metropolitan and regional Victoria until June 2028

Eligibility

This program supports older people living in Victoria who:

  • are 65 years or older, or 50 years or older if they are Aboriginal or Torres Strait Islander
  • live in the community (not in aged care)
  • are experiencing mild to moderate symptoms commonly associated with frailty, such as:
    • reduced mobility, strength, balance, endurance
    • unintentional weight loss
    • fall or unplanned hospital presentation in the past 12 months.

People who meet the above eligibility criteria may also include people who:

  • have recently been discharged from hospital or are about to be discharged from hospital
  • are experiencing social isolation and/or loneliness
  • from culturally and linguistically diverse backgrounds and/or have limited verbal and written skills in English
  • have multiple other health and/or mental health comorbidities
  • are at risk of poor health outcomes due to economic and social impacts.

Referrals

Referrals can be made by health professionals, pharmacists, hospitals, community and social care providers, multicultural and Aboriginal services, or by the individual or their support person. Referrals can be directed to any of the participating community health service providers delivering the program.

The program will be available at the following community health services by early 2026.
