Resources for clinicians to build their knowledge of a connected care system and improve health outcomes for their clients.

The Victorian Integrated Care online resources support clinicians who aim to deliver high-quality, integrated care within their local network.

You can complete the modules at your own pace. This lets you learn gradually, reflect on current care pathways, plan improvements and apply what you've learned step by step.

Access to resources

To get started, you will need to create an account via the Victorian Integrated Care Training websiteExternal Link .

Once logged in, you can access to 6 modules:

  • Introduction to the Victorian Integrated Care Model
  • The patient centred health system and integrated care
  • Leadership, collaboration and governance in integrated care
  • Quality and safety in patient-centred care
  • Integrated care teams
  • Partnering with patients, their family and carers

Each module includes readings, case studies, activities and videos. These will help you understand the key ideas and practices behind patient-centred integrated care.
