If the parent/carer tells you they have lost their child's Australian Immunisation Register (AIR) Immunisation History Statement:
To enrol
Do not enrol the child until the acceptable AIR Immunisation History Statement is provided, unless the child is eligible to enrol under the 'support period' (see below).
Ask the parent/carer to contact the AIR.
Give the parent/carer AIR contact details, and tell them to request a statement from the AIR.
The parent must obtain the AIR Immunisation History Statement and provide a copy to your service.

If the family is experiencing vulnerability or disadvantage
To enrol
Consult the parent to find out if they qualify for the 16-week support period. If so, the child can commence at your service while your service takes reasonable steps to obtain the AIR statement from the parent/carer.
You should give the parent/carer with information on how to obtain an AIR statement.
