About care and recovery coordination

Care and recovery coordination is one of the treatment streams for people who require coordinated and longer-term care within the Victorian alcohol and other drug treatment system.

Care and recovery coordination integrates treatment and care pathways across the treatment system for people with the highest need or who are at greatest risk. It provides additional individualised and flexible support that supplements other alcohol and other drug treatment services over a longer period.

Care and recovery coordination can include pre-care support to clients on waiting lists (by telephone, face to face or online) and continues throughout the clients’ treatment and for up to 12 months after commencement of treatment.

Eligibility for care and recovery coordination

People are referred for care and recovery coordination when, through intake or comprehensive assessment, an alcohol and other drug service provider identifies that they require complex intervention (assessed as Tier 5).

People eligible for care and recovery coordination typically present with behaviours and/or conditions that:

  • place the individual at high risk to self, to staff and/or the community
  • are identified as requiring a long-term supportive service
  • require residential treatment.

Objectives of care and recovery coordination

Care and recovery coordination involves a person and their family in coordinating their care and setting recovery goals. Treatment and care is planned with the objectives of meeting recovery goals, supporting access to other health and human services and maximising opportunities for meaningful social and economic participation.

Care and recovery coordination services

Care and recovery coordination services are located across the metropolitan and rural and regional catchments in Victoria, with flexibility to service multiple sites through face-to-face and telephone modes.

Service providers operate 52 weeks a year during standard business hours (Monday-Friday) and may provide after-hours service.
