Victorian AOD self-completion form

Comprehensive assessment supports drug treatment providers to determine the level and type of treatment and support required by a presenting client. The Victorian AOD self-completion form is completed by clients at the beginning of comprehensive assessment for drug treatment, to provide baseline data including on substance use, health and wellbeing. The comprehensive assessment tool is designed for use in conjunction with the self-completion form, and from information collected at intake, to ensure that a client's comprehensive treatment needs are adequately understood.
