Community Health – Health Promotion funding is provided to agencies across Victoria (primarily community health services) to develop and implement prevention and health promotion initiatives.
It is recommended that initiatives have a focus on primary prevention (aimed at preventing problems from occurring in the first place), and are developed and delivered at a scale that can impact the health and well-being at a population level.
These documents will assist organisations funded through the Community Health – Health Promotion program with planning, implementation and reporting for the 2021-2025 planning cycle.
