Authority to release medical information

This form gives authority to the police to obtain relevant medical information relating to the occupational violence incident if the employee required medical attention. Delay in completing this form may disadvantage the investigation and the prosecution of the offence may be unable to proceed. The original of the completed form needs to be given to the relevant medical practitioner/hospital and a copy provided to the investigating police officer.
