Your information is stored within the department’s secure systems for management and administration purposes only and is only accessible by Mental Health and AOD System Management.
All data is subsequently deleted from our systems each month.
If you do not provide all required information
If you do not provide us with the information requested in this form, we will not be able to provide you with a hard copy of your selected patient rights statements. However, you may still access the digital copies of the patient rights statements.
