    The department is collecting your personal information. The department was established to help Victorians stay safe and healthy and deliver a world-class health system that leads to better health outcomes for all Victorians. Our responsibilities include supporting the Health and Ambulance Services, Mental Health and Ageing portfolios.     Purposes of collection

    The department is collecting your personal information in this Order for mental health publications form for the purpose of providing you with a hard copy of select patient rights statements.

    How we intend to use or disclose your information

    When you complete the form, we collect the following personal information:

    • Name
    • Organisation name
    • Address
    • Contact details, including phone number and email address
    • Details of your order

    We use this information to ship the requested patient rights statements to you.

    We do not disclose your information to any other third party unless required or authorised under law. Your information is handled by the department in accordance with the requirements of the Privacy and Data Protection Act 2014 (Vic), together with the department’s privacy policy.

    How your information is stored

    Your information is stored within the department’s secure systems for management and administration purposes only and is only accessible by Mental Health and AOD System Management.

    All data is subsequently deleted from our systems each month.

    If you do not provide all required information

    If you do not provide us with the information requested in this form, we will not be able to provide you with a hard copy of your selected patient rights statements. However, you may still access the digital copies of the patient rights statements.
