This report contains the methodology, evaluation and recommendations of the Neurological Complex Care Project funded by the Department of Health & Human Services in 2006-07 in North and West Metropolian Region, Victoria. The Centre for Applied Gerontology evaluated the data collected throughout the trial phase of the project and their report is included.
