A final report of the Refugee Health Project for WestBay Alliance and Brimbank and Melton Primary Care Partnership. A one-off initiative through the Primary Health Branch of the Victorian Department of Human Services to support improved General Practitioner integration and service coordination for refugee clients. Funds were allocated to each of the eight Primary Care Partnerships where Refugee Health Nurses were situated. The project was envisioned to build on the existing service coordination processes in the Primary Care Partnerships and current agency practice.
