This file contains the form for medical practitioners notifying routine infectious diseases and other conditions in Victoria.

Please note that urgent conditions require notification by telephone on 1300 651 160 (24/7) to the Department of Health on clinical suspicion or initial diagnosis (presumptive or confirmed) as soon as practicable and within 24 hours.
