Improving care for older people: a policy for Health Services outlines how a person-centred approach to the care of older Victorians by health services can benefit the whole community. Settings include the acute and subacute campuses of a health service, as well as the additional programs that a health service provides in the community. The aim is to make sure that older people are being cared for in the appropriate setting for their individual situation, and to empower them and the people who care for them to be involved in decision-making.
