Central Hume Primary Care Partnership integrated protocol 2007

The Department of Health & Human Services Care planning project began in April 2007. The project aimed to progress inter-agency care planning practice across Victoria through Primary Care Partnerships. It includes mapping of current inter-agency care planning work, sharing of good practice models, identifying barriers and enablers, developing a framework to further the implementation of effective interagency care planning which includes the revision of the Service Coordination Plan component of the SCTT. The Paving the Way Project in Central Hume describes protocols and tools to support mental health and well being in the community.
