This frameworks to support discussion around care planning as Victoria moves towards a more integrated system of healthcare planning and care coordination. It builds on work previously undertaken by Inner East Primary Care Partnership (IEPCP), Outer East Health and Community Support Alliance (OEHCSA), Hume Region Primary Care Partnerships and HACC agencies, Central Victorian Health Alliance Primary Care Partnership, HARP (Eastern Health), other PCP agencies and the Department of Health & Human Services. The discussion paper also articulates factors influencing the decision making for the adoption of greater efficiencies in current care planning practice. It pulls together some of the threads of multifaceted and complex healthcare planning issues. It does not provide solutions to the significant issues relating to systemic change required to enhance the current system. In-depth work and dialogue is required to progress many of the issues described in this document.
