Better Living Better Health model of care

Better Living Better Health focuses on improving the care of people with defined chronic diseases and complex needs who frequently use hospitals or are at risk of hospitalisation. This model of care has been developed in the context of: key stakeholder consultation including consumer involvement, successful models internationally, key learnings from the current Better Living Better Health projects and the Department of Health & Human Services HARP chronic disease management guidelines.
