Close-out of Home and Community Care Program for Younger People (HACC PYP) transition to NDIS

Dates have now been confirmed to “close-out” the transition of funds from the Home and Community Care Program for Younger People (HACC PYP) for clients that have transitioned to the National Disability Insurance Scheme (NDIS). This will help create certainty and stability for clients and service providers.
