Best practice following hospital presentation for self harm

The risk of repetition of self-harm and/or suicide is elevated in the immediate post-discharge period, particularly up to one month post-discharge. This literature review examines suicide risk monitoring systems, risk assessments and interventions during the post-discharge period.
