Preventing Illness

Prevention, early detection and early intervention can reduce the impact and spread of diseases among Victorians and reduce avoidable hospitalisations. The government is responsible for a range of initiatives to maintain and improve the health of Victorians.

Immunisation

Immunisation is a proven method of disease prevention, providing high levels of protection against diseases that can lead to discomfort, hospitalisation, disability or death.

Promoting an active, healthy lifestyle

Physical activity and healthy eating have been shown to minimise disease and obesity. They also help build stronger communities by forging networks of people with common interests.

  • getting involved with neighbourhood and local community activities
  • making worthwhile contributions to the community, such as through volunteering.

Hospital Admission Risk Program (HARP)

The Hospital Admission Risk Program has been established to achieve better health for Victorians who have complex health needs. The HARP program targets patients who have conditions that do not always require intensive medical treatment and who attend hospitals frequently seeking help to manage their conditions.

Some of the conditions targeted by HARP include chronic conditions such as heart failure, respiratory illness and diabetes. The HARP program helps people become more independent in managing their condition so they can safely and comfortably remain at home.
