Hospital Admission
Risk Program (HARP)
The Hospital
Admission Risk Program has been established to
achieve better health for Victorians who have complex
health needs. The HARP program targets patients who have
conditions that do not always require intensive medical
treatment and who attend hospitals frequently seeking
help to manage their conditions.
Some of the conditions targeted by HARP include chronic
conditions such as heart failure, respiratory illness and
diabetes. The HARP program helps people become more independent
in managing their condition so they can safely and comfortably
