Overview
Surgical mortality - Due to the introduction of the Victorian Audit of Surgical Mortality (VASM), the Victorian Surgical Consultative Council (VSCC) ceased receiving direct reports of surgical mortality from the 1st January 2008. The VSCC strongly encourages all surgeons to participate in the VASM. For information on reporting to the VASM please email vasm@surgeons.org.
The VSCC will continue to receive reports regarding surgical morbidity.
Surgical morbidity refers to injury in association with or as a result of surgery. The Council encourages reports of any significant morbidity.
Identifying information on this reporting form is confidential to the Chairman of the Consultative Council. This enables the Chairman to contact the reporting clinician should additional information on a reported incident be required.
Subsequent review by the full Council is by case number only, as all identifying information is deleted prior to the full Council reviewing an individual case of surgical mortality or morbidity.
