Referral information

To be Eligible for VPRS the young person should be aged between 0-18 and:

  • Have had an acquired injury, medical/surgical intervention or a functional impairment
  • Have moderate to high complexity rehabilitation needs and / or require a time limited and goal oriented program.
  • Require an interdisciplinary team approach to achieve their rehabilitation goals.

Who can refer to VPRS

VPRS will accept referrals from any of the people listed below, however we do like to make sure that your local Paediatrician and/or General Practitioner is involved in this process, so we may ask you to get a medical referral prior to your first appointment. This allows us to have a clearer picture of medical conditions and treatment options. We accept referrals from:

  • Paediatricians
  • General Practitioners
  • Health Professionals
  • Paediatric Service Providers
  • Families

How to Refer to VPRS

Referrals can be made by emailing, faxing or posting your referral to the most relevant Regional Coordinator. We do prefer you to use our Referral Form (PDF file, 31kb) and complete all relevant information.

If you are unsure if VPRS is the right service for a child please don’t hesitate to contact one of our regional coordinators.

If you are a community therapist and have a client attending one of our clinics or receiving intervention such as Botulinum toxin Injections we also ask you to complete our Community Therapist Form (PDF file, 70kb) which will allow us to liaise more closely with you re times of appointments or post intervention treatment.
