Referral informationTo be Eligible for VPRS the young person should be aged between 0-18 and:
Some stories of the children and their families that have benefited from our services may help with deciding if VPRS can help you, your child or your client. Who can refer to VPRSVPRS will accept referrals from any of the people listed below, however we do like to make sure that your local Paediatrician and/or General Practitioner is involved in this process, so we may ask you to get a medical referral prior to your first appointment. This allows us to have a clearer picture of medical conditions and treatment options. We accept referrals from:
How to Refer to VPRSReferrals can be made by emailing, faxing or posting your referral to the most relevant Regional Coordinator. We do prefer you to use our Referral Form (PDF file, 31kb) and complete all relevant information. If you are unsure if VPRS is the right service for a child please don’t hesitate to contact one of our regional coordinators. If you are a community therapist and have a client attending one of our clinics or receiving intervention such as Botulinum toxin Injections we also ask you to complete our Community Therapist Form (PDF file, 70kb) which will allow us to liaise more closely with you re times of appointments or post intervention treatment. |
