VPRS model of care

The model of care for the VPRS client journey may involve many phases including:

• Access and initial contact
• Initial need identification
• Assessment
• Care planning
• Service Delivery
• Follow up and Re-entry

VPRS services may be delivered as inpatient rehabilitation services, ambulatory rehabilitation services or outreach services.

The VPRS model of care document describes the components of the paediatric rehabilitation model of care currently being developed in Victoria.  It outlines the context of the document, the components of the model of care and relationships and roles to be developed.

The Paediatric rehabilitation Governance Group will oversee any further development of the model of care. This document will be reviewed every 12 months.
