Terms and conditions
All entries must adhere to the following conditions of entry or risk being declared ineligible.
- All entries are to be submitted electronically using the entry forms sent by email from the online submission system by the deadline, 6 April 2011. Late entries will not be accepted.
- The entrant’s chief executive, or equivalent, must endorse each entry prior to electronic submission.
- To be eligible the nominee(s) must work in a voluntary (unpaid) capacity in a Victorian Government funded health care or primary health care organisation, or have conducted unpaid volunteer work.
- Entries will be accepted from Victorian government-funded health care providers and organisations delivering public health programs. Organisations eligible to apply include, but not limited to, hospitals, community health services, mental health services, aged care services, integrated care centres, ambulance services, and volunteer, charity and non government organisations and organisations delivering public health initiatives. Please contact the Secretariat if you are unclear about the eligibility of your organisation.
- Individuals and teams who have previously won a Volunteer Award cannot be nominated again, unless the new nomination focuses on new endeavours or achievements.
- The Department of Health reserves the right to invite entries.
- All entries will be vetted by the Department of Health to ensure the entry complies with relevant terms and conditions.
- The judges reserve the right to not confer an award in any given category.
- The judges’ decision in each category is final and no correspondence will be entered into.
- No hard copy supporting information, such as videos, CDs or posters, will be accepted, unless by prior arrangement.
- Entries should be checked for accuracy and spelling errors corrected prior to submission.
- All entries submitted to the Volunteer Awards assign copyright ownership of the entry to the Victorian Department of Health. The work may be published, used for promotional material, media interviews or as models to support policy development or programs.
- It is intended and agreed that the conduct of the Awards shall not give rise to any legal obligations or duties, valid or enforceable in any way against the Department of Health.
