Important entry information

The 2009 Minister for Health Volunteer Awards nominations must be submitted through the online submission system.

The chief executive from eligible organisations (pdf file, 60kb) can nominate one individual and one team from their organisation. Make sure that the checkbox is ticked on the online submission form indicating chief executive endorsement.

To be eligible the nominee(s) must work in a voluntary (unpaid) capacity in a Victorian Government funded healthcare or primary healthcare organisation, or have conducted unpaid volunteer work.

The entry should provide information about the nominee(s), the reason for the nomination, and the benefits which have flowed to patients, clients, or the community from the nominee’s dedication and service.

The following supporting files are encouraged to accompany the entry.

  • Up to two letters of support, for example from clients, families, team members or paid organisational staff or management.
  • A photo of the nominee(s), preferably in their volunteer role.

Entry steps

  • Step 1: Download the entry form guide PDF and review the submission requirements and conditions of entry for the Awards.

  • Step 2: Determine who to nominate from your organisation – only one individual and one team entry per organisation will be accepted.

  • Step 3: Register your details with the online submission system. An official entry form (MS Word document) and instructions on the submission steps will be emailed to you.

  • Step 4: Prepare the nomination using the official form. Two letters of support and one photo can be submitted with the nomination form.

  • Step 5: Submit the entry by 6pm 24 April 2009 through the online submission system.

Assessment period

There is no specific time period for the Awards but it is preferable that the nominee is currently working in a voluntary capacity at the nominating organisation, or if ceased employment, to have done so in the previous 12 months.

Assessment criteria

The judging panel, which includes the Minister for Health, will review the entries and consider the nominee’s relative merits in relation to their commitment, level of service and the benefits of their work for Victorian consumers, organisation or community.
