Right Care Right Time Right Place: The Development of Implementation Plans for Improving the Patient Journey - Workshops Presentations

During October and November 2008, the Ambulatory and Continuing Care Unit hosted a series of workshops in Bendigo, Shepparton, Geelong, Traralgon, Ballarat and Melbourne, to provide information to clinicians (health service chief executive officers, executive directors of sub-acute services, directors of nursing, middle management in geriatrics and rehabilitation and senior clinical staff) developing implementation plans for improving the patient journey. The workshops had a specific focus on networking, integration and relationships. Presentations on the Sub-acute Planning Framework and the HIP Guidelines were supported by workshop activities stimulating consideration of current and new patient pathways.
