Victorian Maintenance Dialysis Program Activity Targets 2008-09

Block Grant (Capitation Payment)

Activity targets for the Capitation Payment are set for each hub provider on the basis of the previous year’s targets and include a growth component.  The growth component is calculated as a standard rate across all providers based on the growth rate in the average number of patients across the State.

The activity targets are indicative only. Targets and budgets are adjusted on a case-by-case basis to actual activity as reported to AIMS for the year as a Prior Year Adjustment.  Activity targets for 2008-09 are detailed in Table 1 below.

WIES Payments (Case Payment)

The current policy for renal dialysis is to pay hospitals the full value of any maintenance dialysis (MD) WIES based on actual activity.  Where the actual MD WIES exceeds both MD and global WIES targets, the agency’s overall WIES target will be notionally adjusted and the agency will receive the full value of the additional renal WIES.  A similar notional target adjustment will be made where renal activity is less than target. 

WIES targets are set for all services providing satellite and in-patient haemodialysis.  WIES targets for 2008-09 are based on the activity levels for 2007-08 as reported to the VAED.  The targets are an estimate of expected activity for the year. 

Unutilised dialysis WIES are not substitutable for other services and payments will be adjusted at the end of the financial year to reflect actual WIES activity.  WIES targets by region for 2008-09 are detailed in Tables 2 to 7 below. 

The purpose of this arrangement is to ensure that patients will receive MD as close as possible to their home (subject to constraints in capacity, clinical requirements and patient preferences) and that hospitals will not be constrained by competing demands on their budgets.  The risks of uncapped dialysis WIES are limited by the clinical nature of dialysis. That is, patients are only referred to for MD when they have life threatening End Stage Renal Failure.  If treatment is not received, patients die.
