The Registration ProcessRegistration list of information and documents that must be submitted with an application. Once an approval in principle has been obtained, and the premises is ready to operate, the proprietor must then apply to register the premises. Both stages must be completed before the service may commence operation. Application kits for registration of the premises are available below or from the DHS Policy Instruments & Compliance Unit. Application Kits Registration Documents
For National Police Certificates please complete the National Police Check Form and submit directly to the Public Enquiry Office (address located on page 2 of the application). All supporting documentation including payment should be sent with the application. Section 4 of the application should request that the Police Certificate be returned to a nominated facility representative. Once Police Certificates have been returned to the facility from Victoria Police please forward them to the Department with a covering letter to the Manager, Policy Instruments & Compliance Unit, GPO Box 4057, Melbourne 3001.
