The Registration ProcessApproval in Principle The Department of Human Services (DHS) administers the registration system for private hospitals and day procedure centres and is responsible for the processing of applications relating to registration, transfer or renewal of registration, and construction/alterations of private health facilities. To assist proprietors register a private hospital or day procedure centre, the Unit has prepared several application kits to cover the various stages in the registration process. For further information on how proprietors may apply for any of the above applications, or to obtain an application kit please contact Ms Helen D'Offay on telephone 1300 135 760 Before a private hospital or day procedure centre can be registered, the proprietor must first obtain a certificate of approval in principle. Application kits to assist proprietors prepare an approval in principle application are available below or by contacting DHS Private Hospitals Unit. The application kit includes a list of information and documents that must be submitted with an application. Other application kits have been prepared to assist proprietors where they propose to:
Application Kits Approval in Principle Documents
For National Police Certificates please complete the National Police Check Form and submit directly to the Public Enquiry Office (address located on page 2 of the application). All supporting documentation including payment should be sent with the application. Section 4 of the application should request that the Police Certificate be returned to a nominated facility representative. Once Police Certificates have been returned to the facility from Victoria Police please forward them to the Department with a covering letter to the Manager, Private Hospitals Unit, GPO Box 4057, Melbourne 3001. Application for Variation or Transfer of Certificate of Approval in Principle
