Birth defects notification

This Birth Defects notification is used for notification of birth defects to the Birth Defects Register primarily by allied health professionals, i.e. maternal and child health nurses, private practitioners and specialist clinics.

Birth report

This form is designed to collect information on, and in relation to, the health of mothers and babies. The data collected via the Birth Report contains information on obstetric conditions, procedures and outcomes, neonatal morbidity and birth defects relating to every birth in Victoria of 20 weeks gestation or more, or 400 grams or more birthweight.
