Service Coordination Online Learning Module
The Service Coordination Online Learning Module is an e-learning tool to support the practice of Service Coordination in Victoria. Organisations are encouraged to use this module to orientate staff to Service Coordination practice within their organisation and use it for professional development for existing staff.

Course in Service Coordination
A Course in Service Coordination has been approved by the Victorian Registration & Qualifications Authority and is a state accredited course. It may be delivered by Registered Training Organisations as an elective unit for most Community Services Training Package qualifications or a stand alone course.
Registered Training Organisations: Service Coordination Principles
The Service Coordination Principles: Trainer Guides assists Registered Training Organisations to integrate service coordination principles into their training, using case studies to demonstrate the practical application of service coordination principles.
Integrated Chronic Disease Management
Care planning
The unit Implement Goal Directed Care Planning is a competency-based unit that aims to build the capacity of practitioners to undertake person centred care planning, and coordinate care planning, particularly in the context of working with clients with chronic and or complex needs. It is targeted to a broad range of practitioners across the community and health sector and will include definitions of care planning terminology, theoretical frameworks, good practice examples and reference Victorian care planning principles and guidelines.
This training is delivered via the Victorian Education and Training sector by Registered Training Organisations and is a vocational Graduate Certificate level unit.
Currently the training is available across Victoria through the following providers:
- Mayfield Education
- Chisholm Institute of TAFE
- Holmesglen Institute of TAFE
- Victoria University
- Swinburne University
- East Gippsland TAFE trading as Advance TAFE
- Partners in Training Australia
- Wodonga Institute of TAFE
- University of Ballarat
- Skills Training Victoria.
This course provides participants with an opportunity to develop their skills and knowledge to work collaboratively within a multidisciplinary context and and has a focus on person centred approaches and building client self management capacity through collaborative goal setting.
The course will require commitment from participants to attend three workshops and completion of assessment tasks. Participants will be challenged as they explore opportunities to move into a new speciality area of care planning/care coordination.
Higher Education and Skills (formerly Skills Victoria) funding is available to eligible participants.

A number of forums have been held over the past years to bring together organisations progressing ICDM to explore issues of mutual interest.

Australian Disease Management Association (ADMA) Integrated Online Chronic Disease Management Clearinghouse
The ADMA Integrated Chronic Disease Management Online Clearinghouse stores practical resources and tools relevant to disease management services, plus brief explanations of their development and use, which have been uploaded by their authors where others can then search and access them.

Service Coordination Tool Templates (SCTT) Online Learning Module
The SCTT Online Learning Module is being developed by the department to support the implementation of the SCTT 2012 suite of templates. The SCTT Online Learning Module will be released by June 2013 to align with the inclusion of SCTT 2012 into Client Information Management Systems.
The SCTT Online Learning Module will provide a platform for organisations to train their existing and new staff in the use of the SCTT 2012 suite of templates. This will allow for a standardised SCTT training across the state. SCTT training can therefore be targeted to align with the release of SCTT 2012 in an organisations software application so that SCTT training is available as SCTT 2012 becomes available to users.
Integrated Health Promotion - resource kit
The Integrated Health Promotion resource kit is a practice guide to support agencies and organisations to plan, deliver and evaluate effective integrated health promotion programs.
