Community Health Plans

The Primary Care Partnership Strategy outlines the following priorities for the 31 PCPs:

  • Partnership Development
  • Integrated Health Promotion
  • Service Coordination
  • Integrated Chronic Disease Management

Each of the 31 Primary Care Partnerships are required to produce a Community Health Plan for 2006-2009 with annual updates as required. The Community Health Plan produced by each PCP focuses on the above priorities and includes planning and reporting mechanisms for each priority.

Each PCP is encouraged to tailor their plan to suit issues relevant to their geographic location (catchment).

PCPs work together with their Members and use the Community Health Plan as a tool to achieve mutually beneficial outcomes for the community.

Young Carers Information Kit: PCP - South West
This PCP - South West member kit has been prepared for professionals working with and supporting young people. It is designed to provide information and strategies to identify and assist young people who may be acting in a caring role. Based upon The Australian Bureau of Statistics 1998 report "Disability, Aging and Carers: Summary of Findings", there are 388,800 young people under 26 years of age in Australia (representing 6% of this population) providing care for family members or friends with a disability, mental illness, drug and alcohol issues, chronic condition or who are frail aged.
