The Service Coordination Tool Templates (SCTT) are used by over 600 health and human services. They are currently being revised to improve their use in a broader range of programs and to better meet the needs of consumers. The current revision is the third revision since the introduction of the SCTT in 2002.
The scope of the revision and the governance structure is the most complex undertaken to date. This reflects greater policy alignment of various government programs towards improving the coordination of services for their clients. The attached diagram shows the governance structure and the flow of information for recommendations for improvements.
The SCTT 2012 Revision Project Governance Structure outlines the review and endorsement process for improvements to the SCTT, SCTT User Guide and recommendations for the Victorian Service Coordination Practice Manual. The various committees and groups comprise senior representatives from across government, the sector and relevant peak bodies.
The SCTT 2012 revision is linked to and supports the revision of the Victorian Service Coordination Practice manual which is led by the Statewide PCPs Chairs’ Executive.
There are two ways to participate in the SCTT 2012 revision process. They are by:
participating in working and reference groups; and
providing feedback to consultants which have been contracted for selected areas only.
The current cycle of revision of the SCTT, known as the SCTT 2012 revision process, has commenced. In addition to addressing the high priority changes that were unable to be addressed through the SCTT 2009 revision process, a number of major pieces of work are being undertaken. They are recommendations for the VSCPM and improvements to the SCTT and the SCTT User Guide to better:
meet the needs of people with chronic disease and complex needs
facilitate feedback processes
meet the needs of Aboriginal and Torres Straight Islander consumers
meet the needs of people with intellectual disabilities
support the provision of the carer’s information necessary to support best practice service coordination for consumers
Also to be addressed through the SCTT 2012 revision process, are the:
Broader implementation of the VSRF template and the development of GP practice supports
Undertaking of a joint project (subject to confirmation) between the Department of Health’s Integrated Care Branch and the Department of Education and Early Childhood Development’s Child, Youth and Family Division to improve the VSCPM, the SCTT, and the SCTT User Guide to better meet the needs of children and their families
Improvement of the function of the full suite of the SCTT, based on feedback from the sector, and the better alignment of the SCTT with state and national data standards, including data standards developed by the National E-Health Transition Authority
Review and update of the VSCPM, the SCTT, and the SCTT User Guide implementation products including the service coordination self-paced training module
Further development of implementation products for vendors of client management software applications including an updated functional specification, data dictionary and data model, HL7 messaging standards, and decision support
