Development of Tool Templates for Initial Needs Identification and Care PlanningPage content: Overview | Contact | Download document OverviewThe Department has contracted a consortium led by the Australian Institute for Primary Care, and including HDG Consulting, the Centre for Health Services Development at Wollongong University and the Health Issues Centre, to develop statewide tool templates for Initial Needs Identification and Care Planning. In consultation with Primary Care Partnerships and other key stakeholders (including consumers), these tools will be developed and piloted over the remainder of 2001, before being mandated for use by PCP member agencies in 2002. This information resource briefly sets out the purpose and scope of the tool template development project and the broad process and timelines for undertaking the work.
