Service Coordination & ICDM Survey 2009

The Service Coordination & ICDM Survey 2009 is now closed. Thank-you for your participation in this quality improvement process. The results will be available through your Primary Care Partnership (PCP) in December 2009. The statewide results will be also available on this website after analysis of the data.

Service Coordination Survey 2008

The Service Coordination Survey is a component of the Continuous Improvement Framework that supports the Victorian Service Coordination Practice Manual. Over 200 agencies responded to the survey, this includes a total of 636 programs. More information about the survey and the results can be found in the documents below.
