Service Coordination & Integrated Chronic Disease Management Survey 2009Page contents: Contact | Download documents The Service Coordination & Integrated Chronic Disease Management (ICDM) Survey provides an opportunity for agencies to reflect on their current practice, identifying areas of good practice and areas requiring further focus locally, regionally and at a state-wide level. This is the second year that the Service Coordination survey (Part A) component has been implemented; allowing agencies to benchmark their practice over time. It is the first year ICDM questions (Part B) have been included. An increased number of agencies (n=230) responded to the 2009 survey, this includes a total of 773 programs.
