This Manual has been developed as a reference guide for managers responsible for leading and managing Service Coordination and for practitioners involved in the implementation of Service Coordination.

The purpose of this manual is to define practices which support Victorian service providers to work together in a coordinated way to give consumers a streamlined and integrated response.
