Overview

Within the context of the Better Access to Services framework and to add value to the service coordination work undertaken by the four Hume Region Primary Care Partnerships, this HACC funded project sought to engage with service providers and develop a consistent understanding of the work practices and processes that staff undertake when using the terms care coordination, care planning and case management as well as identify the protocols that would support this work in the Hume region.
