Consumer consent form in community languagesPage content: Overview | Contact | Download document OverviewHealth information is sensitive and consumers need to be confident that they have made an informed decision regarding the disclosure and sharing/release of their information. The Health Records Act 2001 places limits on when and how an organisation can share information. The essential principle is that a consumer's health information may only be used or disclosed for the purpose for which it was collected, or for purposes as authorised by law. It is critical, therefore that the consumer understands what they are consenting to and for what purpose, and that they can refuse to provide their consent without it impacting on the referral. When using the 19 translated consent forms health practitioners need to ensure that appropriate practices are in place. This would include the use of interpreter services when deemed suitable. The consumer consent form has been translated into 19 community languages and will be translated into further community languages at a later date. ContactContact information for further enquiries.
