Literature Review of Effective Models and Interventions for Chronic Disease Management in the Primary Care SectorPage content: Overview | Contact | Download document OverviewThis review has identified a number of multicomponent, multidisciplinary interventions and models of care,evaluated by controlled trials in the primary health sector,which have resulted in improvements in the processes and outcomes of chronic disease management.While the review found that relatively little research had been undertaken to evaluate packages and settings of chronic illness care in the primary healrh sector, a range of effective models were identified for the care of the elderly patient with chronic illness, for specific chronic diseases including asthma, diabetes, cardiac disease and hypertension, and for chronic mental illness.
