Positive outcomes from Primary Care Partnerships

Reports that describe the positive changes to consumers since the implementation of the Primary Care Partnership Strategy are now available.

The reports were launched at a lunchtime seminar at the Department of Human Services, on Wednesday 4 December.

Speakers included:

  • Mr Martin Hawson, Manager, Aged and Disability Services, Mildura Rural City Council, who spoke about coordinated service delivery and practice supported by electronic referral and the use of a comprehensive service directory in the Loddon Mallee Region.
  • Ms Nicky Melville, CEO, Upper Hume Community Health Service, who spoke about an integrated health promotion program that is helping more than 400 isolated older people across seven towns in north east Victoria manage their pain and reduce the incidence of falls though a vast array of exercise classes.
