Project overview
Development of the unit Implement Goal-directed Care Planning commenced in early 2010 as part of a Department of Health Care Planning Project.
During the later part of 2010 we conducted a pilot Vocational Graduate Certificate level course with Mayfield Education which was provided to a diverse range of participants from across the North East and Wellington & South Coast Primary Care Partnership. The course commencing in 2012 has been significantly redeveloped based on the feedback and recommendations of our pilot project.
The unit, Implement Goal-directed Care Planning will address gaps in skills and knowledge in care planning for a broad range of practitioners across the community and health sector and will include definition of terminology, good practice examples and reference to Victorian care planning principles and guidelines.
This course provides participants with an opportunity to develop their skills and knowledge to work collaboratively with clients in a care coordinator or care planning role.
The course will require a commitment from participants to attend three workshops and completion of assessment tasks. Participants will be challenged as they explore opportunities to move into a new specialty area of care planning/care coordination.

Information for Applicants
Agency co-funded places
As each training place is a co-funded place, agencies will be asked to contribute $200.00 for each practitioner attending the training. For successful applicants, all other costs will be funded through the care planning project.
Funding arrangements can be made for groups and special provisions will be made for those agencies who do not have capacity to contribute. To discuss or negotiate special arrangements, agencies should contact Kate Boucher on 90969407.
The course is applicable for those community services and health practitioners who require a high level of knowledge or skills in the area of care planning in the following contexts:
- provision of specialist services to clients with complex and diverse needs
- act as a resource for other practitioners
- provide practice supervision to staff
- work intensively with clients.

- You will be notified by email that your application for a funded (or co-funded) place has been received
- All applicants will be advised if they have been successful or not
