Overview
The Service Coordination Tool Templates have been developed to support
Service Coordination practice by assisting with identifying the initial
needs of clients and providing a vehicle to collect and share core client
information in a consistent way. Guidelines have been developed to provide
information about the purpose and use of the tool templates but the practice
change required will be determined by local agreements between agencies
known as practices, protocols, policies and systems.
