Overview

A range of Department of Human Services program areas and their service sectors have now implemented service coordination and a number of other Programs will be mandating the use of the Service Coordination Tool Templates by 2006. More detail about Service Coordination implementation is available below.

Victorian Statewide Referral Forms – A guide to using the electronic service coordination tool template in medical director

The Dandenong District Division of General Practice (DDDGP) and The South East Primary Care Partnership (SEPCP) recently developed a self directed learning CD aimed at improving GP use of the Victorian Statewide Referral Form, VSRF (GP version of the service coordination tools). The CD was a result of a project, which was funded under the GP service coordination small grants initiative.

The CD titled 'Victorian Statewide Referral Forms – A guide to using the electronic service coordination tools template in medical director' uses a powerpoint presentation with narration to assist GPs, who use medical director software, to access the tools.

The presentation provides a brief overview of service coordination and the use of the SCTT. It then takes GPs through a step by step process in how to access, complete, and save the forms.

A copy of the CD has been sent to all PCPs and GP Divisions within Victoria for use and promotion.

A copy of the presentation can be obtained from the Dandenong District Division of General Practice website.

The presentation is located in the resources section of this site (under Forms, Templates & Proformas). It is a Powerpoint presentation, if you do not have Microsoft Office you can open the presentation using the free Powerpoint viewer program on the website – see link in Resources section.

General Practitioners and service coordination

Small grants for service coordination with general practice

Since 2002, the Department of Human Services has made available a limited funding pool for small grants of up to $10 000 to enable Divisions of General Practice , Primary Care Partnerships and their member agencies to work in partnership to involve general practice in local service coordination arrangements. Service coordination implementation information resource (May 2003)

This document provides an update of the initiatives that will support electronic referral between primary care service providers and others across Victoria and assists Primary Care Partnership member agencies in making informed decisions regarding implementation of electronic referral.

Personal Alert Victoria (PAV)- program & service guidelines

The Guidelines have been developed for use by managers of PAV designated assessment agencies and their staff undertaking PAV assessments, PAV service providers, policy makers, and DHS Regional and Head Office Staff. The guidelines aim to assist with an understanding of PAV operational policies and procedures.
