Service Coordination Tool Templates & Victorian Statewide Referral FormPage contents: Overview | SCTT 2009 Forms | SCTT 2009 User Guide | Training | VSRF OverviewThe Service Coordination Tool Templates (SCTT) is a suite of templates developed to facilitate and support service coordination. The SCTT support the collection and recording of initial contact, initial needs identification, referral and coordinated care planning information in a standardised way. Using the SCTT can improve communication between service providers, the recording of information generated by screening and assessment processes, information sharing, and the quality of referrals and feedback between service providers. This can assist service providers to share relevant information to support better outcomes for consumers. SCTT 2009 FormsThe most efficient way to use the SCTT 2009 in an electronic format is to have the templates incorporated into your organisation’s client information management system in accordance with the published specifications. Please check with your software vendor. Information for software vendors to implement the SCTT 2009 is available from the Information Management & Information Communications Technology section of this website. Where it is not possible or feasible to incorporate the SCTT 2009 into your organisation’s client information management system, there are a number of alternatives that you may wish to use including PDF and interactive word. SCTT 2009 forms are available as Adobe Acrobat files. SCTT 2009 forms are available as interactive word templates. The interactive word templates were developed using 'verdana' font. To download the templates successfully so that the format is maintained you will need to be using a version of ‘Microsoft Word’ that has the verdana font available. If you are not sure whether you have verdana available on your system you can check by opening a blank document in word and then going to the font menu to see whether verdana is available as a font option. If it is not available we suggest you speak to your IT support so that they can install the verdana font for you. When you are saving the templates to your system for ongoing use we suggest that you create a new folder within your templates folder called 'Service Coordination' and save them into that folder as a '.dot' so that you can access them easily and save changes more efficiently. Creating a ‘Service Coordination’ folder will ensure that a 'Service Coordination' tab appears amongst the other template category tabs when opening a new word document. If you are not familiar with how to save documents in your templates folder you may need assistance from your IT support. TrainingInformation sessions covering the SCTT 2009 revision were delivered in all regions across the state during August 2009. The seventeen sessions that were provided using a 'Train the Trainer' model were filled to capacity with 493 participants in total. The SCTT 2009 training evaluation report includes a statewide summary and regional specific evaluations. This report highlights the greater diversity of program areas participating in the training this year and the readiness of the participants to provide SCTT 2009 training to their collegues. Victorian Statewide Referral Form (VSRF)The Victorian Statewide Referral Form (VSRF) is a straightforward, easy to use template for referrals from general practice. The purpose is to provide a standardised, quality referral from general practice to state funded health care providers. The Victorian Department of Health and General Practice Victoria promote and support general practice to use the VSRF as a replacement for the multitude of service-specific referral forms.
