Service coordination practicePage content: Overview | Service Coordination Survey 2008 | Contact OverviewThe "Victorian Service Coordination Practice Manual" defines the common practices, processes, protocols and systems which support Service Coordination across Victoria. It replaces all local and regional Service Coordination Practices, Processes, Protocols and Systems (PPPS) manuals (16 different manuals were in existence in May 2006). The manual:
The manual includes a pull-out poster that describes the consumer’s path through Service Coordination. The manual is supported by two resources: the "Good Practice Guide for Practitioners" and the "Continuous Improvement Framework" (note that the framework is in draft form. A final version will be available after development of the Service Coordination implementation survey). A list of frequently asked questions is also available for your information. Download documents
Primary Care Partnerships may also have local protocols to complement the Victorian Service Coordination Practice Manual and information about these may be found on each of the specific PCP webpages. Service Coordination Survey 2008The Service Coordination Survey is part of the Continuous Improvement Framework. The survey will collect information that will measure the depth and spread of use and the full impact of Service Coordination.
